How to make a complaint with the Office of Licensing and Monitoring

If you have a concern about an issue or event regarding the operation of a Child Placement Agency (Treatment Foster Care, Independent Living or Adoptions) or Residential Child Care program (i.e. child and youth group home) you may file a complaint with the Office of Licensing and Monitoring. The Office of Licensing and Monitoring is required to investigate all complaints made against private foster care providers licensed and/or monitored by our office. The Office of Licensing and Monitoring will initiate a response to complaints from any source about the private foster care provider within 24 hours. You may file a complaint through one of the following:
